Players use the D-pad to move and climb ladders, the A button to jump, and the B button to attack (breathing fire). The Select button switches between items, and the Start button uses the currently selected item. The game features a variety of functional items that add strategic depth: ladders for reaching high places, red shoes to boost movement and jumping, small bombs to destroy walls or attack enemies below, boomerangs for ranged attacks, masks to detect hidden treasure chests, and wings that allow temporary flight to navigate difficult terrain in later stages.

Critics and players praised Karnov for its hardcore design and innovative mechanics. The item-switching system introduced a strategic layer, enemy placement and hit detection were challenging, and the protagonist’s distinctive look and personality left a lasting impression. The controls have some weight to them, requiring players to adapt to Karnov’s jump and movement inertia. The soundtrack is infused with Central Asian motifs, and although the NES port’s graphics are simpler than the arcade version, they still convey the exotic atmosphere.
