故事主軸圍繞一名主角展開，他因被誘騙參與假打工而遭神秘組織進行人體實驗，變成人造吸血鬼「魔骸者」。在即將被處理之際，他被名為刻風雫的少女所救。她屬於更高階吸血鬼「夜咬一族」，並與守護秋葉原的自警團合作，一同調查魔骸者暴走與神秘組織的真相。

採用多重結局設計，在過程中的選擇會影響劇情走向，包括與不同角色的合作關係、事件處理方式等，讓每次遊玩都可能導向不同結果，提升重玩價值。

在《導演剪輯版》中，最大亮點是新增「卡蒂路線」。這條原本在2013年版本中未能完整收錄的角色劇情，如今被重新整合並擴展，加入全新劇本與配音內容，補完故事並導向所謂的「真正結局」。此外，也完整收錄所有先前推出的DLC內容，包含各種服裝與裝備合作要素，讓玩家可以自由搭配角色造型，進一步強化個性化體驗。

Set in a faithfully recreated version of Akihabara, the game delivers a surreal yet grounded experience where players navigate a lively urban district filled with shops, side events, and eccentric characters. The city feels alive, allowing players to freely interact with NPCs, shop for gear, trigger random encounters, and uncover hidden conflicts beneath the surface.

At the core of the gameplay is the series’ signature “strip action” combat system. Enemies, known as artificial vampires called Magaimono, are vulnerable to sunlight. To defeat them, players must engage in battle by targeting and tearing off their clothing, exposing them to sunlight and ultimately defeating them. This mechanic is both strategic and humorous, as different clothing pieces affect stats, and players can equip outfits to customize abilities and appearance.

The story follows a protagonist who is lured into a fake part-time job and turned into a Magaimono through human experimentation by a mysterious organization. Just as he faces disposal, he is saved by a girl named Shizuku Tokikaze, a member of a higher vampire lineage known as the Night Eaters. Together with a vigilante group protecting Akihabara, they investigate the growing threat of artificial vampires and the conspiracy behind their creation.

One of the defining features of the game is its branching narrative system. Player choices—such as which characters to ally with and how to resolve key events—directly influence the story’s direction, leading to multiple endings. This adds replay value and allows players to experience different perspectives within the same world.

The Director’s Cut version introduces a major addition: the long-awaited “Kati Route,” centered on the heroine Kati Raikkonen. This newly implemented storyline, originally planned but not included in the 2013 release, has been fully developed with new dialogue and voice acting, offering a fresh narrative path that leads to what is considered the game’s “true conclusion.”

In addition, all previously released downloadable content is included, giving players access to a wide variety of costumes and equipment from the start. These range from stylish outfits to crossover-themed gear, enhancing both gameplay variety and visual customization.
